Physician Burnout Survey

You are being asked to complete a survey entitled “A Survey Evaluating Burnout, Health Status, Depression, Alcohol and Substance Use, and Social Support Among Webinar participants”

The survey should take 10 – 15 minutes to complete. The purpose of this study is to assess the prevalence of risk factors for job burnout, depression, substance use, and health problems among individual clinical care providers participating in an on-line Webinar.

This study is being conducted by Dr. Steve Hyman, Principal Investigator with the Vanderbilt University Medical Center. During the survey, you will be asked questions about Substance Use, Health Status and Depression. An example of a question you will be asked is “Have you felt calm and peaceful?”

All answers are private and will not contain any information that could identify you.  Participation in this survey is voluntary, and there are no negative consequences whether or not you choose to participate in the survey. You will not directly benefit from your participation in this study; however your participation will help us assess the prevalence of risk factors for job burnout.
