2007 ASA Survey of Fees Paid by Commercial Carriers

Please download the questionnaire and give it to your practice administrator or chief billing person.

Only one response per practice, please.  If you have questions, please send an email to Mail@ASAwash.org with the subject "Fee Survey." 

The results will appear in the Practice Management column in the August ASA NEWSLETTER.  Results of the last survey were published in the August 2005 NEWSLETTER.

Feel free to distribute copies of the questionnaire to other practices and encourage them to complete one copy each, too.
