ANESTHESIA FEE SURVEY

Please complete the 2005 ASA survey of fees paid by commercial carriers - deadline extended to June 27th

Have you found the 1997, 1999, 2001 and 2003 surveys of commercial/nongovernmental payments for anesthesia services useful?  The 2005 survey will be even better with your participation.  Please download it here and return it to Sharon Merrick in the ASA Washington Office by email s.merrick@asawash.org or by fax, (202) 371-0384.

You do not need to identify yourself or your group (but you are certainly free to do so).   To make sure that we only receive one response per anesthesia group, we ask that you give the questionnaire to your administrator or billing service.  These individuals may already have completed and returned the survey.  To the rest of you:  thank you in advance!

The deadline is June 27, 2005.  The results will appear in the August ASA Newsletter.
