Application for Distribution License for CROSSWALK^®, Reverse CROSSWALK^® or Relative Value Guide^®

 

ASA has initiated a process for authorizing certain uses of CROSSWALK^®, Reverse CROSSWALK^® and/or Relative Value Guide^®. As the sole owner of copyright rights in these publications, ASA will now require a license to use or reprint content from these publications to create any product or publication intended for sale or distribution in the U.S. or abroad. Licenses will apply to use of the 2008 editions, which are now available for purchase, and future editions of these publications.

Each license will grant a non-exclusive right to use content from the then current editions of CROSSWALK ®, Reverse CROSSWALK^® and/or Relative Value Guide^® only. Distribution may be in print, electronic media, or over the Internet. Reasonable royalties to ASA may be required with reports to be submitted quarterly. 

Please note that using or reprinting content from these publications requires use of the nomenclature developed and copyrighted by the American Medical Association in its Current Procedural Terminology, 4th Edition (CPT-4) publication. A separate license with the AMA for use of the CPT ^® codes contained within the ASA publications is required.
