| Nominations
for Distinguished Service Award |
he
House of Delegates has established policies governing
the selection of a recipient for ASA’s Distinguished
Service Award. Procedures for the submission
of nominations and selection of a candidate for 2006
will be as follows:
1. Any member of ASA or a component society may submit
the names of individuals for consideration for this
award.
2. Nominations must be submitted on the nomination
forms, which may be obtained from the ASA Executive
Office, together with a current curriculum vitae.
3. Nomination forms should be submitted to:

4. Nomination forms must bear a postal mailing date
prior to August 1, 2006.
5. The committee will review the names of nominees
submitted and recommend to the House of Delegates
the name of no more than one candidate. Selection
of a candidate shall require a two-thirds vote of
the full committee. The Distinguished Service Award
may be given to an ASA member for outstanding clinical,
educational or scientific achievement or contributions
to the specialty and/or exemplary service to the Society.
6. No officer of ASA shall be eligible for selection.
7. Should the committee select a candidate to enter
into nomination in the House of Delegates, the name
of the candidate shall not be disclosed until placed
in nomination before the House of Delegates.
8. Final selection of the recipient of the Distinguished
Service Award shall be made by the House of Delegates
by secret ballot and shall require a two-thirds vote
of those seated in the House of Delegates.
