Update (5/5/17): CMS releases new tool for eligible clinicians to check their MIPS participation status.
CMS recently released a new tool on its Quality Payment Program website that allows eligible clinicians (ECs) to use their National Provider Identifier (NPI) to check whether they are exempt from MIPS. The website also notes that if ECs are included in MIPS, they may be exempt with the second review of eligibility determinations in late 2017.

The Centers for Medicare & Medicaid Services (CMS) will notify anesthesiologists and other clinicians of their MIPS eligibility beginning in late April through May. Practices will receive letters detailing the participation status of each eligible clinician within a Taxpayer Identification Number (TIN) or practice. Practices should be aware that the letter will arrive from their Medicare Administrative Contractor (MAC) that processes their Medicare Part B claims.
Clinicians are considered eligible for participation in MIPS if they have more than 100 Medicare patients per year and bill more than $30,000 in Medicare Part B allowed charges. MIPS eligibility is based on Medicare Part B claims from previous years.
Clinicians will receive notification of their patient-facing and hospital-based status from CMS at a later date.
