ASA is the sole owner of copyright rights in CROSSWALK^®, Reverse CROSSWALK^® and Relative Value Guide^®. A license is required to use or reprint content from these publications to create any product or publication intended for sale or distribution in the U.S. or abroad.

Each license will grant a non-exclusive right to use content from the current editions of CROSSWALK^®, Reverse CROSSWALK^® and/or Relative Value Guide^®. Use of content from past editions may also be permitted as deemed appropriate. Distribution may be in print, electronic media, or over the Internet. Reasonable royalties to ASA may be required with reports to be submitted quarterly.

Please note that using or reprinting content from these publications requires use of the nomenclature developed and copyrighted by the American Medical Association in its Current Procedural Terminology, 4th Edition (CPT-4) publication. A separate license with the AMA for use of the CPT^® codes contained within the ASA publications is required.
